The US Consulate in Guangzhou has a set of examples that they show adopting parents to help them fill out their forms correctly. We would have found it very useful to see these forms before showing up for our interview. They claim that these examples are now included in packets sent to adopting parents and to agencies, but I have not heard from anybody who received these samples in advance. For that reason, I have scanned the set I obtained from the Consulate, and am making them available for download and printing. You should have blanks of each of these three forms in the packet you received from the Consulate after you got your I-171H from the INS. These images are large, so be prepared to wait for several minutes for each page.
Please remember that these examples are for the most straightforward cases. Your specific adoption may differ. If you have questions, please address them to your agency, who can then bring them up with the Consulate. Sarah Metzger has assured me that the consulate is also happy to directly answer questions from parents, contacting them by telephone or fax.

The US Consulate has stated that if the adoption certificate has your child's American name included, and if her passport has her American name on the "Observations" page, then they will issue her visa in her American name, saving a name change before applying for naturalization. Our facilitator wasn't sure what the "Observations" page was, and the Security Office that issues the passport was suspicious of the request, fearing fraud. You may wish to take a copy of this next image to show your facilitator if you wish to try to get your child's new name on her passport.
